Ear Flush – $30

Forms – School forms, ODSP forms, School vaccine forms, LTD forms, Work FAF forms. Price can range from $100.00 and up depending on the length and type of form.

MTO/Driver’s Medical/3rd Party Physical Examination – $160.00

School/camp physical examinations – $50.00

Non-resident visit fee –  $200.00
